PHRM 776 - Population Health Management


Prerequisites, PHRM 501 PHRM 511 PHRM 531 PHRM 592 PHRM 622 PHRM 622L PHRM 631 PHRM 641 . The goal of this one-credit elective course is to familiarize students with population health management within health care system and the role pharmacists have in this type of delivery of care. This course will also focus on motivational interviewing and direct patient interactions. The course will prepare students to think about pharmacist roles outside of the traditional community setting. The last 3 weeks of the course will require you to complete experiential hours (total of 6 hours) assisting with population health tasks for a local health systems. Letter grade. (Offered every trimester.) 1 credit
